We’re sorry to learn you have not yet been able to speak live with your Medical Provider or received the letter your medical provider might have sent you in the mail. We hope you will speak with your Medical Provider live or receive the original letter soon. However, we provide the following text, which is the same information that would be conveyed by your Medical Provider when you were prescribed to participate in the Remote Services care program.
“ Recently, an exciting new program became available to patients enrolled in Medicare or a Medicare Advantage plan. I have prescribed this program for you, because of the healthcare improvements reported for patients like you.
The program is called Remote Patient Monitoring, Remote Therapeutic Monitoring, and Critical Care Management. I’ll refer to these as Remote Services for short. It only takes a few minutes of your time, but allows our nursing support team and Medical Providers to be more intimately involved in your healthcare each month between your office visits.
In the Remote Services a medical staff member familiar with your health will be assigned to you and will continually review your health data to evaluate potential risk factors. Changes in measurements like: weight, blood pressure, and glucose levels can provide early indications of important changes in your health.
Remote Service offer us the ability to coordinate a higher level of care with less cost to our patients.

You are going to be contacted by our Support Team. They will ask you to officially enroll in our Remote Services care program. Your enrollment in Remote Services care is very important to your health.
