However, we provide the following text, which is the same information sent in the letter to you by your medical provider when you were prescribed to participate in the CCM & RPM care program.
“ Recently, an exciting new program became available to patients enrolled in Medicare or a Medicare Advantage plan. I have prescribed this program for you, because of the healthcare improvements reported for patients like you.
The program is called Critical Care Management, CCM for short and Remote Patient Monitoring, RPM for short. It only takes a few minutes of your time, but allows our nursing support team and medical providers to be more intimately involved in your healthcare each month between your office visits.
In the CCM program a medical staff member familiar with your health will be assigned to you and will call to discuss your care at times you indicate are convenient for your schedule. In the RPM a medical staff member familiar with your health will continually review your health data to evaluate potential risk factors. Changes in measurements like, weight, blood pressure, glucose can provide early indications of important changes in your health.
CCM & RPM offer us the ability to coordinate a higher level of care with less cost to our patients.
